VOLUNTEER ASSUMPTION OF RISK AND RELEASE

Description of Volunteer services to be performed:

Neighbor Visitations, Community Clean-Up & Events

Location(s): Grant Park Christian Academy, Grant Park, Highland Pines, Palmetto Beach

I certify that I am physically and emotionally able to participate in these Services. If I have medical conditions that may potentially impact or be impacted by. the Services, I have listed them below. In an emergency, I hereby authorize Faith Action Ministry Alliance staff or volunteers, and/or emergency and medical personnel to make emergency medical decisions for me.

I UNDERSTAND AND HEREBY AGREE TO ASSUME ALL THE RISKS WHICH MAY BE ENCOUNTERED BY MY PERFORMANCE OF THE SERVICES CONDUCTED FOR ON MY BEHALF OF THE MINISTRY, including transportation to and from said events. In consideration of my being permitted to participate in the Services described above and other valuable considerations the receipt of which is acknowledged, I hereby AGREE TO RELEASE, DEFEND, INDEMNIFY, AND HOLD HARMLESS FAMA, New Testament Worship Center, First Missionary Baptist Church of Highland Pines, Iglesia Restauracion Cristiana, Crusader Place and Amazing Love Ministry and its agents and employees from any and all past, present, and future, known and unknown liabilities, actions, causes of action, claims, expenses, personal injuries, and damages, INCLUDING THOSE CAUSED BY THE NEGLIGENCE OR FAULT OF THE MINISTRY, ITS LEADERS, EMPLOYEES, OR VOLUNTEERS, and including without limitation, interest, penalties, court costs, attorney's fess and expenses resulting from or on account of injury to my child (if applicable), myself, or my property in connection with the Services. I FURTHER RELEASE all claims brought by or through me, including claims for loss of consortium and all similar claims based on relationships with other people. I EXPRESSLY AGREE that this release, waiver, and indemnity agreement is intended to be as broad and inclusive as permitted in the State of Florida and that if any portion hereof is held invalid, it is agreed that the remainder shall, notwithstanding, continue in full legal force and effect. I ALSO AGREE that any controversy or claim, by or through the party signing this release, arising out of or relating to the Services shall be settled by binding Christian arbitration conducted by the National Center for Life and Liberty or another Christian arbitrator, and judgement on the award may be entered in any court having jurisdiction thereof This release contains the entire agreement between the parties hereto and the terms of this release are contractural and not mere recitals.

I FURTHER STATE that I have carefully read the foregoing assumption of risk and release agreement and know the contents thereof and I sign this document as my own free act. This is a legally binding agreement which I have read and understand.

As a volunteer, I abide by all applicable standard, policies, and procedures of FAMA and to fulfill the volunteer responsibilities to the best of my ability. I understand that I will receive no monetary benefits in return for the volunteer service I provide and that FAMA may terminate this agreement at any time without prior notice and with or without cause.

ENTITY OWNERSHIP OF PROPRIETARY INFORMATION: Any reports, histories, studies, tests, surveys, manuals, instructions, photographs, negatives, blueprints, plans, maps, data, system designs, computer code, or any other documents and drawings prepared or during preparation by the volunteer while engaged in the performance of those services set forth in the "VOLUNTEER OVERVIEW" shall be the exclusive property of FAMA and all such materials shall be remitted to FAMA by the volunteer upon completion, termination, or cancellation of service. A volunteer shall NOT willingly allow or cause to have such materials used for any purpose other than performance of the volunteer's service under this agreement without written consent of FAMA.

CONFIDENTIALITY: A volunteer shall keep all information confidential, in whatever form, produced, prepared, observed or received by the volunteer to the extent that such information is confidential by law.

I have read a copy of the volunteer responsibilities and I ascertain that I am physically able to complete the tasks to which I will be performing.

I hereby grant Faith Action Ministry Alliance Inc. (FAMA), a 501(c)(3) nonprofit community organization, permission to use photographs and video footage granted during my association with FAMA for editorial, promotional, and digital purposes. This permission is granted in perpetuity and applies to all media formats, including print, online and social platforms. Proper credit must be provided to FAMA for each image or video used. Any commercial use of these materials requires prior written consent from FAMA. For further information or to request additional permissions, please contact (813-670-4164).

Should you discontinue volunteer service with us, for whatever reason, we would appreciate feedback on your experience volunteering with us by completing an exit survey. The exit interview will be in digital form. If you aren't completing mandated community service hours, you are under no obligation to complete this Exit Interview.
